Which medicines are on the Bridge list?
Medicare.gov lists three products for the temporary program. The formulation matters, especially for Zepbound.

Who may qualify?
The Bridge is available to certain adults with Medicare drug coverage (Part D), including qualifying standalone drug plans and Medicare Advantage plans with drug coverage. A prescription for weight management, lifestyle-program participation, and prior authorization are required. CMS sets clinical criteria based on body mass index at the start of GLP-1 therapy and, at lower thresholds, certain accompanying conditions.
For adults 18 or older, the CMS clinical criteria include one of these starting points:
- BMI 35 or higher, or
- BMI 30 or higher with heart failure with preserved ejection fraction, uncontrolled hypertension, or chronic kidney disease stage 3a or higher, or
- BMI 27 or higher with prediabetes, a previous heart attack or stroke, or symptomatic peripheral artery disease.
Those criteria are more specific than simply having Medicare or obesity. CMS says people already receiving a GLP-1 through Part D, or who have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH, are ineligible for the Bridge even if they meet its BMI criteria. What does $50 actually mean?
Medicare.gov says an eligible Bridge beneficiary pays a $50 pharmacy copayment for a one-month supply, either 28 or 30 days depending on the medicine. The Bridge operates outside the regular Part D benefit. That $50 does not count toward the Part D deductible or yearly out-of-pocket limit, and Extra Help does not reduce it.
Key distinction:The $50 figure describes qualifying Bridge fills. A GLP-1 covered by a person's regular Part D plan can have different coverage and cost-sharing rules.
Why the 2027 headlines are confusing
Two CMS programs appear in the news. The Medicare GLP-1 Bridge began July 1, 2026 and now runs through December 31, 2027. CMS says the separate BALANCE Model will not launch in Medicare in 2027. That change did not end the Bridge. The programs have different rules and should not be treated as the same coverage promise.
