What should I bring to the evaluation?
Ask the office what information to bring and how to submit any existing records for review. The physician will determine whether additional testing is needed. Laboratory testing is charged separately from the evaluation.

Eligible visits can take place by telehealth while you are physically in Nevada. Confirm that telehealth is suitable for your appointment when scheduling. The physician may recommend an in-person assessment or additional information before making a treatment decision.
